Application Info and Instructions

Feeder School Students:

Fifth grade students attending one of Revere’s five feeder elementary schools (Marquez, Palisades, Kenter Canyon, Canyon, Topanga) do not need to fill out an application. Your child is automatically accepted. Revere will receive all the necessary information directly from the elementary school. Please make sure the information that you have provided to the elementary school is current and correct.

We will mail a letter in the spring asking for confirmation that your child will be attending Revere in sixth grade. The purpose of the letter is to assist us in planning for the incoming sixth grade class.

Residents:

If you live in Revere’s attendance area, your child will automatically be accepted. PROOF OF RESIDENCY IS REQUIRED.

To check if you live in our attendance area, go to www.lausd.net and click on “Parents/Guardians”, and then click on “School Finder” (under “More Resources”).

You may come to the attendance office to fill out the enrollment packet anytime after December 1st, 2009.

Please bring copies of the following documents with you. Please note: We will not make copies of documents

Child’s Birth Certificate Current utility bill, in parent/guardian’s name. (DWP or Gas Co. ONLY. We do not accept phone or cable bills.) Immunization records Transcripts/report cards Standardized test scores Most recent IEP (for Special Education students only)

Siblings:

If you have a child currently attending Revere in the sixth or seventh grade (not in eighth grade) during the 2009-2010 school year, then your fifth grader will automatically be accepted for the 2010-2011 school year.

Please come to the attendance office to fill out the enrollment packet anytime after December 1st, 2009.

Please bring copies of the documents listed under “Residents” with you. Please note: We will not make copies of documents.

Charter School Applications:

Incoming sixth graders: If your child does not meet any of the above criteria, you may still apply to our charter school. Charter applications will be available in the Main Office beginning December 1, 2009 through February 1, 2010 or be printed online from the Revere website. You must turn in the completed application to the Attendance Office ONLY. A receipt will be given when the completed application has been received. Your receipt is your proof of application. Any application received after the February 1, 2010 deadline will be placed at the end of the wait list, in the order it was received.

Selection for admission will be made through a lottery. The lottery will be drawn on Wednesday, March 24, 2010 at 7 pm in the Auditorium. Letters of acceptance to Revere or of placement on a wait list will be mailed on Friday, April 9, 2010. Please notify Revere of any address changes so that we may effectively communicate with you via US mail.

You do not need to bring any documents with you at the time of application. If your child is selected for admission through the lottery process, we will require the necessary documentation at that time.

The number of Charter school applicants that Revere accepts depends on the amount of feeder, resident, sibling, PWT and PSC students we have to enroll.

Please Note: Due to the change from “First Come, First Admitted” to a lottery selection process, there is no need to be the first to turn in an application. As long as you turn in an application between December 1, 2009, and February 1, 2010, your child will be in the lottery for admission.

Incoming seventh and eighth graders: Depending on class sizes, we may accept incoming seventh and/or eighth graders. Please fill out charter application (available December 1, 2009) and turn into Attendance office. You do not need to bring any documents with you at the time of application. If your child is selected for admission, we will require the necessary documentation at that time. You must get a receipt, as it is your proof of application. Applications for incoming seventh and eighth grade students will be placed on a list in the order they are received, and are not subject to the sixth grade lottery.

2010-2011 Choices Brochure for applying to either
Magnet, PWT or PSC program

-District Transportation Programs-

Magnet Schools/Centers:

LAUSD Choices brochure will be mailed to the home of all LAUSD students by mid-November. Brochures will also be available at local school sites in the main office, selected local libraries and at Local District Offices. After reviewing the Choices brochure, complete the application and mail it as directed prior to the January deadline. Students are randomly accepted into Magnet programs based on the number of priority points. The number of students selected is based on the number of available spaces at a particular school. Any student who sends in an on-time application, but is not selected, will be placed on a waiting list at that school. Magnet applicants will be notified by mail during the month of May by the Office of Student Integration Services as to whether they have been accepted or placed on a waiting list. Students who are selected receive District-paid transportation to and from school. Please call the Office of Student Integration Services at (877) 462-4798 or (213) 241-4177 for more information.

Permits with Transportation (PWT)

Student candidates who live in selected areas of the city predetermined by the LAUSD Office of Student Integration Services may apply to attend the Revere Charter School. The application process is the same as for the Magnet program. Complete the application in the Choices brochure, and check the appropriate box indicating application is for the PWT program. Please note: School assignments are made by the District, which means that your child may receive PWT, but not necessarily to Revere. Student candidates are selected through a central selection process conducted by the LAUSD Office of Student Integration Services. Selected students for participation in the PWT program will be notified by mail during the month of May. Selected Students will receive transportation by the District. Please call the Office of Student Integration Services at (877) 462-4798 or (213) 241-6572 for more information.

Public School Choice (PSC):

The district offers the Public School Choice (PSC) program to students enrolled in a Program Improvement (PI) school, or a School-At-Risk of being identified for PI in the following school year. The list of Program Improvement schools and Schools At-Risk for PI is provided in the Choices brochure. Parents desiring to access this program should use the application in the Choices brochure and check the appropriate box indicating application is for the PSC program

Please note: It is the District’s responsibility to identify the Non-PI schools and provide the school choices to parents based on the student’s grade level and resident school. Parents who exercise the option to transfer through the PSC program will receive Non-PI schools of choice by mail during the month of May. Students in the PSC program will receive District-paid transportation. Please call the Specially Funded Programs Branch at (213) 241-6990 for more information.

*Please be aware that Revere does not participate in “Open Enrollment”. All enrollment at Revere is determined through the above policies and procedures.*
