2017-2018 7th Grade Charter WAIT LIST
This application if for NON-RESIDENT'S of Paul Revere's district boundary who would like to apply to be on the 7th grade charter wait list. The charter lottery application window was November 1, 2016 through Friday, January 13, 2017. This application is being submitted after the lottery window and will therefore be on the wait list. If space is available students will be contacted in order of the date the application was received.
Your receipt is your printed copy of this application. The receipt number is the last four digits of your IP address. You should print a copy for your records prior to submitting.
This application is ONLY for NON-RESIDENT'S APPLYING FOR THE 2017-2018 7TH GRADE CHARTER LOTTERY. If you are a resident of Paul Revere's district boundary, please DO NOT fill out this application.
I am not a resident of Paul Revere's district boundary. I am applying to the Charter School 7th grade wait list. My acceptance will be determined as space permits after all the lottery applicants have been offered the open positions.

----1) AGREEMENT: Only submit one application per student. If multiple applications are submitted for a student, the student will be placed at the end of the wait list. -----2) TRANSPORTATION TO REVERE: Paul Revere Charter Middle School DOES NOT provide transportation. Students enrolled through the Charter are ineligible to receive transportation to and from Revere on a school bus through LAUSD. It is the parent/guardian's responsibility to provide transportation. -----3) ACKNOWLEDGMENT: Under penalty of perjury, by electronically signing below, I/we agree that the information contained herein is accurate and we agree to adhere to Paul Revere Middle School expectations. Information falsely provided in the application and/or students failing to follow any Charter policies and procedures may result in the student losing his/her Charter status and be returned to his/her school of residence, if applicable. We acknowledge and understand the Agreement and that student must maintain standards of achievement.* -----4) COMMUNICATION: I understand that all communication regarding the 7th grade wait list results and confirmation will be done via email.
