Application to the Charter School 2008-2009 School Year
Feeder School Students:
Fifth grade students attending one of Revere's five feeder elementary schools (Marquez, Palisades, Kenter Canyon, Canyon, Topanga) do not need to fill out an application. Your child is automatically accepted. Revere will receive all necessary information directly from the elementary school.
We will mail a letter in the spring asking for confirmation that your student will be attending Revere in sixth grade. The purpose of the letter is to assist our planning for the incoming sixth grade class.
Residents:
If you live in Revere's attendance area, your child will automatically be accepted. PROOF OF RESIDENCY IS REQUIRED.
To check if you live in our attendance area, go to www.lausd.net and click on Parents/Guardians, and then click on School Finder (under More Resources).
You may come to the attendance office to fill out the enrollment packet during our designeted enrollment time.
Please bring copies of the following documents with you. Please note: we will not make copies of documents.
Child's Birth Certificate
Current utility bill, in parent/guardian's name.
(DWP or Gas Co. ONLY. We do not accept phone or cable bills.)
Immunization records
Transcripts/report cards
Standardized test scores
Most recent IEP (for Special Education students only)
LAUSD Student ID number (Obtain from current LAUSD School)
