Dear Parent or Guardian,
All students entering 7thgrade after July 1, 2014 must bring proof of a Tdap Booster given on or after their 7th birthday.
District Nursing Services has immunization clinics throughout the District. These clinics are open during school hours. Please visit www.dns.lausd.net to access clinic schedules online. No appointment is necessary. Bring student’s immunization record.
You may obtain a Tdap Booster for your child from your Health Provider or from any Los Angeles County Clinics (low cost or no cost).

There is NO grace period for obtaining this vaccine. If verification is not provided to the school on or before the first day of school, the student will be excluded immediately. The student must be excluded from school attendance until the proof of having received the Tdap booster is provided to the school.
Parents who choose to exercise their right to request an Exemption due to medical or personal reasons, should be provided with the California Department of Public Health “Personal Beliefs Exemption” (PBE) form. These forms are available at the schools and should be signed by the parent at the school site in front of school personnel. Please remember that the clear intent of the law is that the PBE option be used for deeply held personal beliefs, not for convenience, such as a parent misplacing an immunization record.
